電話申請需要輸入的資料，依序
1. register number or confirmation number
2. 2 digits of test month and last 2 digits of test year
3. 2 digits of birth month and 2 digits of birth day
4. institute or fellowship number(s) and department number(s)
5. credit card number
6. 2 digits of month and last 2 digits of year of expired date of credit card
PS:有些學校沒有註明他們的department code或是要你寄到他們的Admission office，結果我加發時候就亂打department code，然後對方會回信說他們還沒收到成績。
然後我就去查資料，原來是如果沒有特別指明，TOEFL的code要用99，而GRE的code要用0000，因為事先沒查清楚，害我要多加發三間TOEFL (Scripps, Duke, U of Minnesota)跟兩間GRE (Scripps, Duke)，媽的～～～萬惡的ETS!!
